IMT interview preparation for 2027 recruitment

2027 round Applications open 22 October 2026

Use the confirmed national Round 1 dates to plan, but treat the detailed interview format as provisional for 2027 until IMT Recruitment publishes its autumn update. The latest IMT-specific pages currently describe the 2026 process and say 2027 application information will be updated in autumn 2026.

Recruitment rules, scoring and interview structure can change. Recheck IMT Recruitment and Oriel before acting on any date or format described here.

The latest published IMT interview format

At the last review of this guide, IMT Recruitment's detailed pages described the 2026 interview as an online, national, multi-station interview. They described two 11-minute stations, preparation time before each station and approximately 30 minutes of interview activity overall.

The published scoring areas cover application and achievements, suitability for IMT, ethical/professional/governance reasoning, communication, clinical investigation/diagnosis/management and patient handover. Candidates start with a two-minute verbal presentation about achievements relevant to training in internal medicine. The clinical scenario develops as additional information is provided and ends with a one-minute handover.

This is the latest published IMT-specific structure, not a guarantee of the 2027 format. IMT Recruitment says its 2027 application information will be updated in autumn 2026 and that the application process is expected to be largely unchanged.

The latest published scoring framework uses two interviewers per station, scoring each area from 1 to 5. For 2026, a score of 3 represented satisfactory performance at the expected level, while appointability rules also considered individual low scores and the raw interview total. Recheck the 2027 scoring page rather than building a strategy around the previous thresholds.

Prepare the capabilities the format is trying to observe

01

Achievements

Select evidence relevant to becoming an internal medicine trainee. Make your contribution, impact and circumstances clear without inflating claims.

02

Suitability

Show informed motivation, knowledge of the work, physicianly interests, learning behaviour and evidence that your development fits the programme.

03

Ethics and governance

Recognise principles, stakeholders and risk; use GMC guidance, communicate openly and describe proportionate escalation.

04

Clinical reasoning

Structure assessment, stabilisation, investigation and management while communicating uncertainty and asking for appropriate help.

05

Handover

Communicate the situation, background, assessment and required action concisely enough that the receiving clinician can act safely.

2027 update

Round-specific changes

Replace this uncertainty with the official 2027 structure, scoring and technology requirements when IMT Recruitment publishes them.

An eight-week IMT interview preparation schedule

Start earlier or compress the plan according to your circumstances. The order matters more than the exact week: verify the process, build accurate evidence, practise domains separately, then combine them under time pressure.

Week 8Map the official domains

Read the current IMT Recruitment pages, audit your evidence and identify the areas that need new preparation rather than more repetition.

Week 7Build the achievements presentation

Select relevant achievements, confirm every claim is accurate and practise a two-minute version that remains understandable without slides.

Week 6Prepare suitability evidence

Create concise examples for motivation, teamwork, communication, learning, managing uncertainty and commitment to physicianly work.

Week 5Rehearse ethics and governance

Use unfamiliar scenarios to practise identifying principles, stakeholders, immediate risk, escalation, documentation and learning.

Week 4Practise clinical structure

Rehearse verbal assessment, prioritisation, differentials, investigation and management at the expected level, using current clinical guidance outside this coaching page.

Week 3Tighten one-minute handovers

Practise concise SBAR-style handovers that make the concern and requested action unmistakable.

Week 2Run full timed circuits

Combine stations under realistic time pressure. Review communication as well as content and rotate unfamiliar prompts.

Week 1Reduce novelty

Review official instructions, identification and technology requirements; keep practice short, targeted and focused on recurring weaknesses.

Do not use AI, automated technology or unpermitted assistance during the real interview. IMT Recruitment states that candidates found using AI or automated technology during interview will be disqualified and may be referred to a professional or regulatory body. PrepMed is for private preparation before the interview.

Six IMT interview questions to practise

  1. 01

Give an overview of the achievements most relevant to becoming an internal medicine trainee.

    Answer outline

Build a two-minute presentation with a clear theme. Select a small number of relevant achievements, state your contribution and impact, acknowledge your circumstances appropriately and finish with how the evidence supports readiness for IMT.

  2. 02

Why are you suited to Internal Medicine Training?

    Answer outline

Connect informed motivation for physicianly work with evidence of clinical development, teamwork, communication, handling uncertainty and a realistic understanding of the programme.

  3. 03

A colleague's conduct creates an ethical or patient-safety concern. How would you respond?

    Answer outline

Identify the competing duties, protect patients, establish facts, communicate respectfully, use GMC principles and appropriate escalation, document where required and follow up.

  4. 04

How would you approach an acutely deteriorating medical patient?

    Answer outline

Prioritise immediate safety, structured assessment, monitoring, early help, focused differentials and investigations, appropriate initial management, communication and repeated reassessment. Follow current guidance in real practice.

  5. 05

Give a concise handover for a patient whose condition may worsen.

    Answer outline

Use a recognised structure such as SBAR. Identify the patient without using real details, state the current concern, relevant background, assessment, actions already taken and the exact review or escalation requested.

  6. 06

Tell us about a quality-improvement project and what you would change next time.

    Answer outline

Define the problem, measure and intervention, show your contribution and result, then reflect on stakeholder involvement, sustainability, unintended effects and the next test of change.

Practise the answer, not a script

Try question 1 or question 5 with feedback

Protect your performance on the interview day

Follow the current invitation and IMT Recruitment instructions for identification, arrival, technology and environment. The latest guidance describes an online interview and advises candidates to prepare a reliable setup free from distraction, but the 2027 instructions should be checked when issued.

Keep claims honest and evidence-free of patient-identifiable information. Review your application so you can explain the context and limits of every achievement. During scenarios, make your priorities audible: the panel can score only the reasoning and communication you express.

Before

Remove avoidable uncertainty

Test the specified platform, identification, connection, audio and environment using the current recruitment instructions.

During

Answer the task you were given

Listen for the exact question, state priorities clearly, manage time and avoid forcing a memorised answer onto an unfamiliar scenario.

Practise the answer, not a script

Make the next answer more specific than the last

PrepMed keeps your answer editable, scores it against relevant domains and shows the next improvement to rehearse.
